The company traces its origin to Charter Med, founded in Minnesota in 1974, and was reorganized as United HealthCare Corporation in 1977 under Richard Burke to manage prepaid health plans. It adopted the UnitedHealth Group name in 1998 and separated its benefits and services businesses, laying the groundwork for the modern Optum brand, which was consolidated in 2011. Optum Rx earns spread, rebate and dispensing economics from adjudicating prescriptions for health plan and employer clients, including UnitedHealthcare itself. Optum Health is paid largely on a capitated per-member-per-month basis for populations under value-based arrangements, while Optum Insight sells software, consulting and revenue-cycle services on subscription and fee contracts. Competitive position rests on scale, vertical integration across payer and provider, and proprietary data. Rivals include Elevance Health, CVS Health-Aetna, Cigna and Humana. Revenue is overwhelmingly domestic, with a small international footprint concentrated in Brazil and select European markets.
