Optum Rx earns spread, rebate and dispensing economics from adjudicating prescriptions for health plan and employer clients, including UnitedHealthcare itself. Optum Health is paid largely on a capitated per-member-per-month basis for populations under value-based arrangements, while Optum Insight sells software, consulting and revenue-cycle services on subscription and fee contracts. Competitive position rests on scale, vertical integration across payer and provider, and proprietary data. Rivals include Elevance Health, CVS Health-Aetna, Cigna and Humana. Revenue is overwhelmingly domestic, with a small international footprint concentrated in Brazil and select European markets.
