Gilead was founded in 1987 in Foster City, California by Michael Riordan, initially as an oligonucleotide research company, and shifted decisively into small-molecule antivirals through the 1990s under John Martin's leadership. Milestones include the 1999 acquisition of NeXstar (AmBisome), the 2003 Triangle Pharmaceuticals deal that brought emtricitabine, the launch of Truvada and later Atripla, and the transformational 2011 acquisition of Pharmasset, which yielded sofosbuvir and reshaped hepatitis C treatment. The 2017 purchase of Kite Pharma added CAR-T capabilities, and Immunomedics in 2020 brought Trodelvy. The company remains headquartered in Foster City and operates as a single, integrated corporate structure with global subsidiaries.

Revenue flows primarily through wholesalers and specialty distributors that resell to pharmacies, hospitals, and clinics, with meaningful direct interaction on cell therapies, which require certified treatment centers and complex logistics for autologous manufacturing. Contracting is dominated by rebate arrangements with pharmacy benefit managers, managed care organizations, the Department of Veterans Affairs, Medicaid, and mandatory 340B discounts to safety-net providers. Competitive position rests on formulary access, clinical differentiation in tolerability and dosing, and lifecycle management of long-lived franchises. In HIV, GlaxoSmithKline's ViiV Healthcare is the principal rival; in oncology, competition spans Novartis, Bristol-Myers Squibb, and AstraZeneca. The United States generates the substantial majority of sales, with Europe and Japan the main international markets.
