Elevance Health is one of the largest health benefits companies in the United States, serving tens of millions of medical members primarily through its exclusive licenses to the Blue Cross and Blue Shield brands in fourteen states, including California, New York, Ohio, and Virginia. Medicaid redetermination cycles, Medicare Advantage star ratings and CMS rate notices, and the medical loss ratio floor set by the Affordable Care Act are recurring structural pressures. Concentration in Blue-branded geographies constrains where the company can grow organically, and its Blue Cross Blue Shield Association license imposes governance and antitrust-related covenants. Capital allocation historically emphasizes buybacks, a growing dividend, and bolt-on acquisitions to build Carelon. The company is a large-cap component of the S&P 500 and a benchmark constituent of managed-care indices.

The company traces its lineage to Indiana mutual insurers dating to the 1940s that eventually consolidated as Anthem Insurance Companies. A defining transformation came in 2004, when WellPoint Health Networks, itself the former Blue Cross of California, merged with Anthem to create WellPoint, Inc., then the country's largest health insurer by membership. The combined firm renamed itself Anthem, Inc. in 2014 following the sale of its NextRx pharmacy business to Express Scripts, and later launched IngenioRx as an in-house pharmacy benefit manager. In 2022 Anthem rebranded to Elevance Health to reflect its expansion beyond traditional insurance, and its services businesses were subsequently unified under the Carelon brand.

Elevance sells coverage through three main channels: employer groups purchasing fully insured or self-funded plans, individuals buying on and off public exchanges, and state and federal governments contracting for Medicaid managed care, Medicare Advantage, and the Federal Employees Health Benefits Program. Revenue arises from monthly premiums on risk contracts, administrative fees on self-funded accounts, and, through Carelon, service fees and pharmacy spread and rebate economics. Competitive position rests on Blue brand recognition, provider network breadth and unit-cost discounts negotiated locally, scale in claims processing, and vertical integration through Carelon. Principal rivals include UnitedHealth Group, CVS Health's Aetna, Cigna, Humana, Centene, and Molina; operations are almost entirely domestic.
