The Cigna Group is a health services organization built around two principal businesses. The company traces to Connecticut General Life Insurance, founded in 1865, and INA Corporation, whose Insurance Company of North America dated to 1792. CIGNA was formed by their 1982 merger and progressively pivoted from property-casualty toward health and employee benefits, divesting individual life and P&C lines over subsequent decades. The transformative step came in 2018, when Cigna acquired Express Scripts, then the largest standalone pharmacy benefit manager in the United States, reorganizing the health services businesses under the Evernorth brand in 2020. In 2022 Cigna divested its international life, accident, and supplemental benefits business to Chubb, and in 2024 sold its Medicare Advantage and related businesses to Health Care Service Corporation, sharpening the current two-segment structure.
Revenue reaches Cigna through several channels with distinct economics. Employers, unions, and government payers contract with Express Scripts for pharmacy benefit administration, with fees, rebate retention, and pharmacy network spread forming the earnings mix; specialty pharmacy dispensing of high-cost drugs is a significant component. In Cigna Healthcare, self-funded employer clients pay administrative service fees while Cigna bears clinical management and, under stop-loss, tail claims risk; fully insured commercial and international customers pay premiums against which medical claims are underwritten. Competitive position rests on scale in pharmacy purchasing, integrated clinical programs, and national provider networks, positioning it against UnitedHealth, CVS Health, Elevance, and Humana. Revenue is overwhelmingly domestic.
